Notice of Privacy Practices

How health information may be used and disclosed and how you can access it.

Effective Date: August 17, 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

This Notice describes the privacy practices of Aligned Mosaic LLC ("Aligned Mosaic") regarding Protected Health Information ("PHI"). PHI includes identifiable information relating to your health, health care services, or payment for health care.

Your Rights

You have the right to inspect or obtain an electronic or paper copy of health information maintained about you, subject to applicable law. You may ask us to correct information you believe is incorrect or incomplete. You may request confidential communications in a specific way or at a particular location. You may request restrictions on certain uses or disclosures. If you pay for a health care service or item out-of-pocket in full, you may ask us not to disclose that information to your health plan for payment or health care operations, and we will honor the request unless disclosure is required by law.

You may request an accounting of certain disclosures of your PHI, obtain a copy of this Notice, choose a legally authorized person to act for you, and file a complaint if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

Your Choices

For certain health information, you may tell us your preferences about what we share, including information shared with family members, friends, or others involved in your care. If you are unable to tell us your preference, we may share information when permitted by law if we believe it is in your best interest or necessary to lessen a serious and imminent threat to health or safety.

Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing when authorization is required, and any sale of PHI require your written authorization unless an exception applies. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it.

How We May Use and Disclose Your Health Information

Treatment

We may use and disclose PHI to provide, coordinate, or manage your treatment and related services.

Payment

We may use and disclose PHI to bill and obtain payment for services, including providing necessary information to a health plan.

Health Care Operations

We may use and disclose PHI for activities needed to operate the practice, including quality assessment, compliance, credentialing, business management, and administrative activities.

We may also use or disclose PHI without written authorization when permitted or required by law, including to comply with law; for permitted public-health or safety activities; to report suspected abuse, neglect, or exploitation; for health oversight; in response to qualifying judicial or administrative proceedings; for workers' compensation; for certain law-enforcement purposes; to avert a serious and imminent threat when permitted by law; and to business associates that are required to safeguard PHI.

Mental Health Records and Psychotherapy Notes

Mental health information may receive additional protection under federal or state law. Psychotherapy notes, as defined by HIPAA, generally receive special protection and usually require your written authorization for use or disclosure, subject to limited exceptions allowed by law.

Substance Use Disorder Records

Federal law provides additional protections for certain substance use disorder (SUD) patient records subject to 42 CFR Part 2. When Aligned Mosaic creates, receives, or maintains records protected by Part 2, those records will be used and disclosed in accordance with applicable Part 2 requirements. Certain uses and disclosures may require your written consent, and additional restrictions may apply to the use of Part 2 records in legal proceedings.

Our Responsibilities

We are required by law to maintain the privacy and security of your PHI. We will notify affected individuals as required by law if a breach occurs that may have compromised the privacy or security of unsecured PHI. We must follow the duties and privacy practices described in the Notice currently in effect. We will not use or disclose your information other than as described in this Notice unless you authorize us in writing or another use or disclosure is permitted or required by law.

Changes to This Notice

We reserve the right to change this Notice and make the revised Notice effective for PHI we already maintain as well as information received in the future. A revised Notice will be made available as required by law.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Aligned Mosaic LLC using the contact information below. You may also file a complaint with the Secretary of the U.S. Department of Health and Human Services through the Office for Civil Rights. You will not be retaliated against for filing a complaint.

Additional Client Rights

This Notice addresses privacy rights and responsibilities under HIPAA and related privacy laws. Other rights and responsibilities concerning treatment, informed consent, termination of services, professional duties, and clinical practice policies may be addressed separately in Aligned Mosaic's informed consent, service agreement, client-rights, or practice-policy documents.

Practice note: Mental-health confidentiality, minor-consent rules, and professional disclosure duties can vary by state and circumstance. Aligned Mosaic should ensure its clinical consent and practice-policy documents address any additional Texas and Virginia requirements applicable to the services provided.
